Reviews 1, 2, 3, 4 and the cost effectiveness review were conducted by the School of Health and Related Research (ScHARR), University of Sheffield; Review 5 was by Matrix Evidence; and  Review 6 was by David Leah Associates. Fieldwork was conducted by Word of Mouth Ltd.

We are grateful to all the experts who very kindly gave up their time to prepare a paper and present to the PDG.
