Summary

The National Institute for Health and Clinical Excellence (NICE) has issued full guidance to the NHS in England, Wales, Scotland and Northern Ireland on Photochemical corneal collagen cross-linkage using riboflavin and ultraviolet A for keratoconus.

Description

Keratoconus is a degenerative condition that usually involves both eyes. It affects the shape of the eyeball and causes blurred vision and light sensitivity. Wearing spectacles or contact lenses can help some people who have this condition. Photochemical corneal collagen cross-linkage is an outpatient procedure. First a local anaesthetic is given in the form of eye drops. Then riboflavin (vitamin B2) drops are applied to the eye, and it is exposed to ultraviolet light. This procedure aims to stabilise the outer coating of the eyeball, called the cornea, with the intention of preventing further changes to its shape.

In addition ICD-10 code H18.6 Keratoconus or Q13.4 Other congenital corneal malformations or a combination of the following two codes Q90.-D Down’s syndrome and H19.3A Keratitis and keratoconjunctivitis in other diseases classified elsewhere are assigned.

The NHS Classifications Service has advised NICE that currently these are the most suitable OPCS-4 codes to describe this procedure. The OPCS-4 classification is designed to categorise procedures for analysis and it is not always possible to identify a procedure uniquely.
